Guidelines:

1. Deadline for pre-registration: January 31, 2007.

2. Refund of Registration fees:
If refund is done:
before February 15, 2007 – 50% of the registration fee will be refunded

** There will be NO REFUND after February 15, 2007

3. Registration fees of the following will be waived:
· Senior Members
· Fellow-Life
· Specialty Board Chair and Members
· National Officers, Board of Directors
· PAFP National Past Presidents
· Incumbent Chapter Presidents
· Guest Speakers
· Chair and Co-chair of the Working Committees
· Fellows, Diplomates and CFP Awardees
· Recipients of the following awards:
o Most Outstanding Family Physician in Community Service
o Most Active Secretary
o Most Active Treasurer
o Leadership
4. Fellow-life and Senior Members should confirm their attendance to the convention by submitting the registration form on or before January 31, 2007.
